Science Diet’s Ideal balance dog food has a mail-in-rebate for up to $12.99 in value. Purchases must be made between July 1, 2011 – March 31, 2012. The rebate must be postmarked by April 30, 2012.

You will need to include the following in order to receive your rebate:

• Cash Receipt 7/1/2011 – 3/31/2012
• UPC bar code cut from product package
• Completed Rebate form postmarked by 4/30/2012
